At Trinity Medical Centre we offer multiple different types of appointments and services. If you are looking to book an appointment for one of the following listed below, we encourage you to complete an Online Consultation Contact us about your request.

  • Requests for fit notes and extensions – Did you know that you can self certify for the first 7 days of you illness – follow this link for further information https://www.gov.uk/guidance/ask-your-employer-for-statutory-sick-pay
  • Have a medical or health concern that you feel needs assessing by a medical professional – Your request will triaged and you will be contacted with clinically appropriate next steps. This could be a request for further information/ pictures, sent a booking link to an appointment with a clinically appropriate clinician within a appropriate time frame for your presenting concern, a call from our team to book an appointment or request further information / signposted to other appropriate services
  • Admin Request – This includes – Test Results / Referral Follow Up/  Doctors letter including private insurance and educational letters – for further information on this, please follow the link  https://trinitymedicalcentrehove.co.uk/non-nhs-services/
  • Routine Care Request – Long Term Condition Reviews / Smear tests / Follow up requests for ongoing conditions
  • Repeat prescriptions – You can send your request via the online consultation using this link https://accurx.nhs.uk/patient-initiated/g81070  – Please use the “I have an admin or routine care request” option and follow the instructions. You will then be able to select “Repeat Prescription”
