Care Home Support Services and Self Referrals

Many referrals can be completed by the care homes and do not need to wait until a GP has seen the individual. Please find a list of self-referrals below.  In some cases you might want to seek advice before making a referral. Referrals can be made by any member of the team, ensuring they follow the services referral criteria. Be mindful to provide as much information as possible.

West Hove Community Nurses

The Community Nursing Team accept referrals directly from care homes, if you are a residential care home you can refer directly in writing:- sc-tr.westlocality@nhs.net

If the resident is known to the service and you need advice or support, you can contact also by telephone on 01273 242117 option 2, option 3 (the service operates 8am-8pm Monday-Sunday)
