Diabetes Review

To prevent long term health complications it is important for people with diabetes to have a good understanding and control of their condition, to help with this it is important to attend a review when recommended but at least annually. This review will consist of 2 x 30 minute consultations. The first consultation involves blood test, weight, and foot check. This may be with a Health Care Assistant or a Practice Nurse. The second consultation includes a review of the results and discussion about recommended changes to lifestyle or medications.
