Coronary Heart Disease

Coronary heart disease (CHD) affects the coronary arteries, which are the blood vessels which supply the heart muscle. Sometimes it is called ischaemic heart disease. Ischaemic means blocked blood supply.

As part of your CHD monitoring, you will be contacted by SMS or letter requesting an up to date blood pressure reading. If you do not have access to a blood pressure monitor we have one located in the Ground Floor Reception area at the Practice. Depending on your reading you may be asked to start a further 4 or 7 day blood pressure diary. If you do not have access to a blood pressure monitor we have monitors that can be borrowed.

If you would rather see a Healthcare Assistant to take your blood pressure reading please contact the Practice and we can book you in for this.

You will be invited to book a cholesterol blood test with a Phlebotomist.

It is recommended that patients also take an anti-platelet or anticoagulant, and a statin. If you need are not already on these medications you will be contacted in order to make an appointment with one of our Clinical Pharmacists.

Patients with CHD are eligible to receive a flu vaccination annually during Influenza Vaccination Season. We will contact you when we start to accept bookings for this.
