Sample and Specimen Drop Off

Following recent feedback we have changed the way in which patients can drop off any samples requested by the clinician. The drop off box will now be at the end of the Reception desk (opposite the lift), the last drop off is 3.30 pm – any samples after this time will have to be redone.

Sample submission and information below: Please see below steps to follow:

• Correctly Labelled – All patients must ensure that any samples submitted to the surgery are clearly and correctly labelled with the patient’s full name, date of birth, and the date and time of collection

• Responsibility – It is the responsibility of the person dropping the sample off to ensure accuracy. Incorrect, unclear or missing information may result in the sample being rejected by the laboratory. This means that sample will not be processed and will need to be repeated which can lead to delays in diagnosis and treatment.

• Double Check – Please take a moment to review the steps below to ensure all labels and accompanying forms are correct before placing in the box. This helps ensure safe, timely, and accurate patient care.

Step 1 – Please check all relevant information is filled out clearly on sample pot (Name, Date of Birth, Date & Time Sample Taken) – see photo below

Step 2 – Please make sure the sample is in the bag and is sealed with the correct form attached using the sticky tab – see photo below

Step 3 – Please put bag inside sample box before 15.30 pm each day – See photo below.
