Registering for online service

***Due to the current COVID situation this has currently been turned OFF the only thing you are able to do is request prescriptions, it will be turned back on when we resume normal services***

You can book appointments, order repeat prescriptions and change your contact information online. using our online services module – Emis Patient Access. If you are a registered user then click here to access the EMIS Patient Access system. If you would like to register for the system please read the information below.

Registering for the online system

The registration is simple – With the current COVID pandemic we ask that you send us proof of ID with the below completed form before we give you an access and linkage code. The completed forms and ID can be sent to us via post, email (bhccg.thetrinitymedicalcentre@nhs.net) or can be placed in the black post-box at the gate. Acceptable ID is a photo driving licence or a passport. Parents will be able to have a PIN for their children. Similarly, carers of people with disabilities will be able to take a PIN for them. The ‘Patient Access’ system can be accessed via the web at the link above from any web enabled device including tablets and mobile phones.

Age 16: 

***Patients now over the age of 16 will need to apply to have online access in your own right. As a precautionary measure we will switch off the current access in 1 month. If you decide you wish to apply, please complete the following forms below. Once you have completed the forms please email to the above address with the appropriate identification e.g., passport/driving licence and a utility bill/bank statement. Once you have registered for patient online access you will be able to book appointments, request repeat prescriptions and receive text messages, currently due to COVID you can only request repeat prescriptions.

Please could you also supply us with the telephone number and e-mail address you wish to have on your record. If these details are your parents’ details and you wish for them to have online access on your behalf, please complete the PROXY form and the consent form.***

Over 11 under 16:

Patient and practice benefit

Online access to appointment booking not only saves both time and money for the practice, but also allows reception staff to focus on other key areas and provide a better service to patients. The service has benefits for patients without internet access too – online booking system means that less people will be ringing the surgery, so it makes the practice more accessible over the phone.

The service has proved particularly popular with busy commuters, the elderly and people with disabilities. Families with young children are welcoming the service, as children often become ill at night, and parents can put their minds at rest by booking an appointment straightaway. Then if the child feels better in the morning, the appointment can be cancelled just as easily.

PLEASE NOTE THE FOLLOWING: 

Not all problems can be dealt with in a single 10 minute appointment so we ask that you refer to the following guide when booking appointments online.

Double appointments (20 minutes) are required for the following:

  • ​Gynecological problems/concerns (where an examination may be needed)
  • Joint injection (must have been discussed and agreed with a GP prior to booking)
  • If an interpreter is required to assist you at your appointment. We can arrange this for you via the surgery
  • New/ ongoing mental health concerns

If you arrive for an appointment that does not allow the correct time there is a risk that you may have to re book and not be seen that day.
