New Patients

Registration forms can be collected from the medical centre or downloaded further down this page in advance.

The practice registers new patients, in person only, at the reception desk, between 10.00 a.m. and 5.00 p.m Monday to Friday.

Any person residing within the “practice area” (as described below) is eligible for registration at the practice, depending on vacancies. If we are unable to accommodate you, we will direct you to an alternative local GP practice.

Our practice area is the sea to the south, the eastern boundary is Holland road, continuing into Silverdale Road up to the Old Shoreham Road, eastwards towards the junction of the Old Shoreham Road and Dyke Road and then northwards up to the roundabout at the top of Dyke Road Avenue, then southwards down King George VI Drive until the junction with Nevill Road and then following Nevill Road down to the Old Shoreham Road. Westwards along the Old Shoreham Road until the eastern boundary comprising of a straightline of Bolsover Road, Coleman Avenue and Wish Road down to the sea which extends up to the Old Shoreham Road.

If you live within our practice area please come into the reception area and ask for a registration pack alternatively you can download the necessary forms to register with the practice below.

You can view our practice boundary and check if your postode falls within it by using the NHS catchment area tool. Scroll to the bottom of the map on the catchment tool to enter and check your postcode.

We require Photo ID, Proof of Address (utility bill or bank statement). You will be asked to declare whether you hold a non-UK issued EHIC or an S1 Form.

It would also be useful to have details of any repeat medications you are being prescribed by your current GP.

Please help us to help you

If you are coming with completed new patient registration forms please try and do this after 4pm as this will avoid peak times and reducing your wait time at reception. It will also allow us to prioritise patients wishing to make an appointment first thing in the morning

Change of personal details

If you change your name, address or telephone number please remember to inform the surgery. This may be done either by visiting the surgery, phoning us, submitting this change of personal details form (PDF file) or by updating your record online (you need to be a registered Emis Patient Access user).

More about your NHS number

Each NHS Number is a unique 10-digit number which is printed on your NHS medical card or will from now on be written on any communication you have had from the NHS.

Non-registered patients

An appointment for immediately necessary treatment is only available to people who have incurred an accident or medical emergency at any place in our practice area and who require GP services.  If the resulting condition is potentially life threatening an ambulance should be called using the 999 service.

In addition, if you are away from your normal place of residence and are staying within our practice area for more than 24 hours and less than 3 months, you may also request an appointment.
